Why can't I see certain items in my Evaluation Report?

Look in the Options tab to make sure the appropriate report layout options are checked. Choose the 'Save Settings' of a specific survey using the survey filter, or use the "Save as Default" option, which will save the settings for all surveys that currently do not have settings already saved.
