Multiple Choice
Uses radio buttons
Can add a textbox to answer choice
Can have the last answer be an "opt out" ("not applicable" answer choice)
Can have answer be "pre-selected" (default selection)

Can type in "Default" text to populate in the textbox (e.g., email suffix)
Can specify number or text formatting
Can specify a character limit
Each textbox has all the formatting abilities of the regular textbox question
Can have a textbox be "required" (must fill out the textbox)
Can have an answer choice be "exclusive" (cannot fill out any other textbox)
